African American women with cardiovascular diseases rely on support from their partners, family members, friends, or neighbors as caregivers to help manage their chronic disease.
As a future nurse leaders, we must be open minded and welcome new ideas that focuses on meeting the needs of the patients. The first step is to acknowledge that health disparities exist.
It is important to always step out of our own perspectives and try to understand from different points of views, instead of making our own assumptions of things.We must accept that there's no glove that fits all. What may work for patient A may not work for patient B.
By educating and advocating for our patients and their families, we empower them with reliable information and resources, which helps improve their health literacy and self care involvement in their care.
At the end, patients just want to be heard, and assisted with their care.
Finally, hospitals can reduce racial and ethnic disparities by providing timely health care, identifying high-need and high-cost patients, and ensuring the continuation of health care in primary care settings post-discharge.
